IMPORTANT HEALTH COVERAGE TAX DOCUMENTS (FORM 1095-C)

If you were a full-time employee or were enrolled in New Horizons’ self-insured health plan during 2025, you are entitled to receive IRS Form 1095-C, which documents your health coverage.

While this form is not required to file your taxes, it is available upon request. To request a copy of your 2025 Form 1095-C, please contact us using any of the methods below:

Forms will be provided within 30 days of receiving your request. This notice will remain available through October 15, 2026.
